adult and end-of-life care.

Your “Medical Home” is a dynamic and busy place.
Quality time with your physician and staff has been a tradition in our practice. This is, in part, very dependent on efficient scheduling of appointments and your role is critical.


Appointments should be made at least 24 hours in advance. Always indicate the specific reason(s) for your visit to allow for adequate visit time. When you ask your physician to address additional issues not scheduled, our ability to adequately assist you is hampered and reduces quality time with other scheduled patients. Please schedule another appointment for additional issues. Plan on arriving 15 minutes before your scheduled appointment. Please let the receptionist know you are there and if your schedule is running tight for additional appointments. Always bring a list of ALL medications, over-thecounter pills, supplements and natural products you are currently using to each visit.
Up-to-date insurance cards and co-pays are required at each office visit. Please plan on showing receptionist current insurance card at every visit.


Please notify us if you cannot make your scheduled appointment. Cancellations are required 24 hours before any scheduled appointment for the convenience and the needs of other patients as well as to avoid financial charges.
